Personal Health Information (PHI) is collected to help provide safe and effective treatment. Your PHI and details of your treatments are collected, stored, and shared per the Personal Health Information and Protection Act, 2004 (PHIPA). Your information is kept confidential and securely and will not be shared with a third party outside of your circle of care without your consent.
If you have any questions about how your personal health information is collected, used or stored, or if you would like to view, correct, receive a copy of, or have me delete your records, please contact Kimberly Kane at kimberly@kindcactusmassage.com. There will be a small fee for receiving a copy of your records.
